Option one:

References from two established providers you have seen recently. Include their name, website and approximate date you last saw them.

Option two:

Your occupation, workplace website, your business card or LinkedIn account, a blank email from your work email (I will not reply), and a selfie of you holding your driver's license.

Feel free to include anything else about yourself you would like to share such as things you enjoy doing (i.e. sports, movies, the outdoors, books, food, wine, etc).
